Membership Descriptions and Application Instructions

  • Description and Application Instructions
  • To Become a Member:
    1. Download and review the membership Application and Agreement Forms.
    2. Complete the Application and Agreement Forms in full.
    3. Print and sign the completed membership Application Form and Agreement Signature pages. Signed by a person in your company/organization authorized to commit to the terms of the agreement on behalf of your company/organization. Note: Only page 6 needs to be signed. Exhibit A is the Confidentiality Form that will be used if any member is involved in a dispute and needs to provide information to a SIGIS investigator; thus page 9 does not need to be signed.
    4. Fax the completed membership Application Form and Membership Agreement Signature pages to SIGIS Membership Services at +1.925.886.3611.
    5. Mail 2 copies of the signed Membership Application and Agreement originals and all attachments to: SIGIS
